About Us

The Lambeth Diabetes Intermediate Care Team was established 8 years ago. During this time the role of the team has expanded and we have been recognised nationally for the innovative, community-based diabetes care we provide.

Services provided by the Lambeth Diabetes Intermediate Care Team

Four specialist diabetes clinics per week in four locations across Lambeth

Early renal disease in type 2 diabetes clinic at Akerman Health Centre on Mondays

Telephone and email advice and support to primary and community care teams

Home visits to people who are unable to attend clinic

Telephone and email support to people with diabetes under the care of our service

Virtual diabetes clinics in GP Practices

Desmond (structured education for people with type 2 diabetes) for Lambeth and SE London residents

Insulin and GLP1 start groups

Professional education to Lambeth primary care and community care teams

Support to Lambeth Care Homes

Enhanced support to people with diabetes and severe mental illness

STEPS (type 2 diabetes prevention programme)

Our ethos

To support Lambeth GP Practices to provide high quality diabetes care

To empower local people to self-manage their diabetes

To provide innovative and responsive diabetes care in the community

To integrate effectively with other care providers including community nursing and secondary care diabetes services

Awards

We were shortlisted for the Innovation category of the national Celebrating DESMOND  Awards 2019 for developing a DESMOND course to be delivered in Spanish to our local Spanish speaking residents. 

The team won Team of the year in the Celebrating Desmond awards 2016 www.celebratingdesmond.org

In 2016 we were finalists in the QiC Diabetes care awards Prevention and Early Diagnosis category for our Steps to prevent diabetes course.

In 2014 the quality of diabetes care provided in Lambeth CCG was highlighted in the London Assembly Health Committee report: “Blood sugar rush. Diabetes Time Bomb in London”

In 2013 the Lambeth Diabetes Intermediate Care Team were finalists in the HSJ/Nursing Times Care Integration Awards.

We were commended in the 2013 QiC Diabetes Care awards in the “Best cross-organisational partnership – reaching out beyond the traditional team” category (in partnership with Guy’s and St Thomas’s NHS Foundation Trust and King’s College Hospital NHS Foundation Trust).
