Community Clinics

Community diabetes clinics are provided at 4 locations in Lambeth - patients referred can choose which location is best for them.

The clinicians at each clinic include a consultant, GPSI, diabetes specialist nurses, dietician and a healthcare assistant. Patients attending the clinics will often be seen by more than one clinician, according to their needs.

Gracefield Health and Social Care Centre

Every Tuesday morning. The clinic is held in the Streatham Suite on the 1st floor.

Gracefield Gardens is located just off Streatham High Road. It is not within the congestion charging zone and there is some non-metered parking for cars on the streets near to the clinic.

Springfield Medical Centre

Every Wednesday morning

Springfield Medical Centre is best approached from Wandsworth Road. It is not within the congestion charging zone and there is metered parking on streets near to the centre.

Akerman Health Centre

Every Friday morning. The clinic is held in the Patmos Suite on the 2nd floor. We also run a specialist renal diabetes clinic on Mondays at Akerman Health Centre.

Parking is metered around the Centre with a few disabled parking outside. The centre is not within the congestion charging zone.

Buses 185, 436, 36 and P5 stop nearby.

West Norwood Health and Leisure Centre

Every Thursday morning.

Address: 25, Devane Way, London, SE27 0DF

There is limited street parking on the side streets close to West Norwood Health and Leisure Centre. The centre is not within the congestion charging zone.

West Norwood train station is opposite Devane Way.
